Care Manager/Case Manager

Position Summary:  The Care Manager is primarily responsible for coordinating support services to participants in the OPTIONS program (65 and older) along with their families/caregivers.  The position major functions involve assessment, resource counseling and support, service
planning, service initiation and monitoring supports for the participants.

Position Qualifications: A Bachelor’s Degree in Social Work or a Social Service related field and at least two years of relevant experience in related services or case management. A minimum of two years experience and knowledge working with the older population is required. The Care Manager must possess a valid PA Driver’s License and access to a vehicle.  Note the hours can vary to include working evenings and weekends as needed.

Primary Responsibilities/Essential Functions:

• The Care Manager will complete face to face visit and initial assessment packet within 2-4 business days after referral.  Of these assessments, 95% will document a correct level of care determination and 100% of substantiating documentation.
• 100% of CM participants will have at least one referral to community resources quarterly.
• The Care Managers will produce a participant driven, strengths-based plan for each consumer that includes participation from family, caregivers, and any other person designated by the consumer.
• The Care Managers will initiate referral to a service provider within 2 business days from date of determination and choice by participant.
• The Care Manager will complete monthly contact by phone or face to face with each participant they support on their assigned caseload.  If increased monitoring is necessary due to regulations, this will be completed as required.
• All documentation will be completed in SAMS within 24 hours of contact with a participant.
• For FCSP participants, the Care Managers will contact 100% of participants each month via phone or e-mail.  The Care Managers will provide each participant a quarterly educational/training opportunity to support their needs.
• Attend all meetings, supervisions and trainings as required.

Salary: $28,000


Working Conditions:

• Work is completed within the office or out in the community at participant’s home or other community and provider agencies.
• Must be able to travel to the various office locations
• Ability to lift and carry light boxes of files and supplies (less than 10 pounds) on occasion
